SOURCE Care Management was established in 2001 and has become a leading provider of primary care enhanced case management services across the state of Georgia. Providing services in most counties across the state, we are committed first to providing our members with quality case management services, as well as providing our employees with an opportunity for personal and professional growth. Our high standards are driven by our core values. We are committed to enhancing all of the communities we serve throughout the state of Georgia by working to improve the lives of our clients. At SOURCE Care Management we put people first!

SOURCE Care Management, LLC doing business as SOURCE and is a wholly owned subsidiary of Home and Community Services, Inc. (HCS). SOURCE provides a central, efficient, and cost-effective manner in which to supply clients and patients with needed home care and related services as a member of Home and Community Services, Inc (HCS).

Each SOURCE Care Management Office is led by an Administrator, under whose leadership, a multidisciplinary case management team provides quality primary care enhanced case management services.

HCS is a Georgia nonprofit corporation organized for charitable purposes within the meaning of Section 501(c)(3) of the Internal Revenue Code of 1986, as amended (the “Code”). HCS is a member in an integrated healthcare delivery system. As a fee contributing member of Community Health Systems, Inc. (CHS), HCS receives assistance with strategic planning, administrative services and other essential functions from CHS.

We will provide the leading model of home and community based care for individuals with chronic and post-acute health and social needs. We will provide support to our clients, their families and caregivers as they navigate, select and receive health and supportive care services.
