New employees are eligible for healthcare benefits on the first day of the month following their hire date. The "paperwork" is all online, so signing up is convenient and easy. Below is a summary of benefits. Benefits Health Care Plans Income Protection Plan Flexible Spending Accounts (FSA) 401(k) Success Sharing Plan Employee Assistance Plan (EAP) Referral Award Program Bonus Plan Time Off Health Care Plans During Open Enrollment each year (and on the first day of the month following a new employee's hire date), employees may select: Medical - two PPO plans (including an HRA PPO plan) and two HMO plans, plus prescription drug programs. Dental - several HMO and PPO plans, which cover expenses for routine oral examinations, cleaning, fluoride treatments and x-rays and other services. Vision - one plan helps pay for eye exams, eyeglasses, contact lenses and frames, and offers laser vision correction procedures at a discounted price. Income Protection Plan Voluntary Life & AD&D offers additional voluntary supplemental employee and dependent life insurance. Basic Life & AD&D is equal to 1x salary. Short Term Disability provides weekly benefits for a maximum of 13 weeks.
