Queens-Long Island Medical Group, P.C. is a healthcare service provider that continually delivers the highest standard of patient-centered primary and specialty care in a friendly, professional environment.

We are currently seeking a few Care Managers for our Queens locations. This position will provide optimum Care Management services with cost effective solutions. Individual will be responsible for collaborating with hospital staff, physicians, patients, insurance carrier, medial office staff, customers & facilities to assess the patient’s healthcare needs across the continuum of care to assure a safe transition.

Job Requirements

Must be a graduate from an accredited nursing program, must hold a current NYS RN license, a min of one (1) year clinical nursing exp. & one (1) year Case Mgmt or related exp. Must be PRI Certified.
