Description Provide physical therapy services to patients/clients including assessment, treatment, program planning and implementation, related documentation and communication.

Provide appropriate timely and comprehensive quality clinical care
Provide and maintain customer satisfaction
Provide complete, accurate and appropriate documentation in accordance with regulatory and specific environment standards
Provide and model leadership for the specific environment and Dependable Home Health
Provide effective and appropriate communication with all persons served
Maintain internal and external standards for continuing education, licensure and/or certification

Requirements

Graduation from an accredited school of Physical Therapy
Valid, current license - AZ State Board of Physical Therapy
CPR certificate
3 years experience
Verbal and written communication skills. Clinical skills that reflect variable experience
Advanced knowledge of Physical Therapy theory and practice. Thorough knowledge of body mechanics and approved lifting techniques
Must be able to transport self to various sites as required. When using own vehicle must have valid Arizona driver's license and proof of insurance that meets the minimum standards of the State of Arizona
