Nursing Positions at Springwell
No nights, weekends, holidays, on-call or hands-on care! Use your assessment and communication skills to provide oversight and clinical eligibility for MassHealth and state home care programs. These programs embrace the Consumers’ Choice in being able to be maintained or the ability to return to the community.
Full-time RN for Comprehensive Screening and Service Module. Assess for Nursing Facility Eligibility for MassHealth payment conducted at Nursing Facility, interviewing the consumer and advocating for the consumer’s choice to return home. Nursing Home/MDS experience preferred.
Per Diem RN to complete initial in home ans annual assessment to determine care plan for any client in need of personal care. Also complete frail elder waivers and perform enhanced care option services evaluations.
Qualifications:
Current MA RN license, a graduate of an accredited Nursing School/ AD Program and 5 years experience or a BSN with 2 years experience.
Previous experience working with elders/ adults with disabilities in the community preferred.
Computer skills needed, including the use of the laptop in the field. Familiarity with SAMS a plus.
Valid driver’s license, clean driving record and reliable vehicle for the ability to make home visits.
