Responsibilities include but are not limited to:
- Providing direct patient care to patients residing in Queens, Bronx & Manhattan
- Coordinators of care, follow-up on changes regarding patient status, medications and assessing for services the patient requires.
- Weekend and Holiday nurses will administer care according to MD orders and report changes to MD.
- See, plan and document their care / visits including the weekend.
- Responsible for submitting billing information accurately and timely to ensure the program gets paid.

Requirements for Home Health / Home Care Nurses include but are not limited to:
- Current and unrestricted RN nursing license in NY
- BSN Degree preferred
- Must have a minimum of two years strong Pediatric or MCH related home care / home health experience with CHHA / LTHHCP
- Must be experienced in OASIS documentation
- Must be eligible to work in the United States
- Availability: Should be available to work at least three days a week and/or have weekend availability
