JOB SUMMARY Manage program operation and administrative services on a day to day basis and provide clinical services to patients according to program protocol. Work under the guidance of the supervising physicians and Chief Nursing Officer. Coordinate and manage direct and indirect patient care of bariatric weight loss surgery patients in the outpatient clinic settings. Provide pre-operative and post-operative clinical care to all bariatric surgery patients. Participate in the screening and assessment of potential patients. Facilitate team conference to determine the eligibility of the potential patients according to program criteria. Provide education to patients on the surgical procedures, self-care practices before and after surgery and on the management of potential surgical complications. Facilitate the care process for individuals preparing for surgery by monitoring the completion of their surgical prerequisites. Has oversight of Bariatric Program staff and front office support staff for Diabetes Education Center. Continuously assess work environment and processes to identify safety risks. Address safety issues identified or brought forward by staff, and resolve as appropriate.

1. Masters Degree in Nursing 2. Possession of a valid, unrestricted California License as a Registered Nurse. 3. Certification by the State of California, Board of Registered Nursing as a Nurse Practitioner. 4. California Nurse Practitioner Furnishing Number 5. DEA Registration Number 6. Possession of National Provider Identification (N.P.I.) number. 7. Minimum of two years nurse practitioner experience in obesity and other chronic disease management in a health care clinic preferred. 9. Current BCLS for the Healthcare Provider required and ACLS certification required *(New hires within 90 days). 10. Excellent interpersonal skills necessary to instruct patients and their families and to collaborate with health team members. 11. Working knowledge of MS Word, Excel and Power Point applications. 12. Experience with electronic health preferred. 13. Experience in facilitating clinical team meetings and chronic disease patient groups.
